SECTION 2: GENERAL CONSENT & LIABILITY

Results & History: I understand that results vary based on skin type, lifestyle, and medical history. I have disclosed all relevant medical info.

Patch Test: I acknowledge that a patch test is available for all services upon request.

Liability: I release Elael’a Esthetics and its technicians from liability associated with any adverse reactions or dissatisfaction due to undisclosed medical conditions or failure to follow aftercare.

SECTION 3: SERVICE-SPECIFIC ACKNOWLEDGEMENTS *

Lash & Brow Enhancements

● Lash Extensions: I will follow the 72-Hour Rule (no water). I will use oil-free products and clean with lash shampoo 1–2x weekly.

● Lifts & Lamination: I will keep the area dry for 48 hours and sleep on my back. I will use a conditioner after 48 hours.

● Clusters & Strips: I will avoid water/oils entirely and will not tug or rub my eyes.

● Henna: I will "let them be" and avoid scrubbing the area to preserve the tint.

Magnetic Tattoo Removal

● Restrictions: I am NOT pregnant/nursing and have been off Accutane for 6+ months.

● Timeline: I have had no Botox/Retinol (4 weeks), surgery (6 weeks), or other removal sessions (10 weeks).

● Recovery: I understand numbing is not used. I will avoid makeup, picking, and sweating for 10 days. I will avoid Retinol/AHAs until all sessions are complete.
